Phoenix House Texas does not have access to any school records of former patients. All school records are maintained by the school district that operated at the facility – either Dallas ISD Hospital Homebound program or Austin ISD.

Requesting Treatment Records

Phoenix House Texas maintains treatment records in compliance with state and federal law, including HIPAA and CFR 42 part 2. In accordance with Texas law, the following information is provided to help consumers understand their rights and available resources.

To request treatment records, please submit a written request to admin@phoenixhousetx.org, with your name and contact information as well as the name, date of birth, and contact information of the individual for whom you are requesting records. A Phoenix House Texas team member will respond to walk you through requirements and next steps for your request.
